Directions
Develop professional speaker notes for the in-person Practicum Project: Health Promotion Collaboration meeting with a minimum of 3 licensed healthcare professionals, following the steps below.

- Use information from Weeks 1 – 3 Practicum Project discussions and assignment to complete the Week 4 Practicum Project: Speaker Note Template.
- Correctly cite and reference ideas and information that come from scholarly literature. Sources should be no more than five years old unless they are historical references.
- Follow rules of grammar, spelling, word usage, and punctuation consistent with formal, scientific writing.
Include the following sections (detailed criteria listed below and in the grading rubric). Answer the questions under each section with explanation and detail. Information from previous weeks may be used.
- Introduction of Self and Practicum Project: In one paragraph introduce yourself: include your name and credentials; your affiliation with Chamberlain University, the program of study, course; and the purpose of the Health Promotion Collaboration meeting.
- Introduction of Topic: In one or more paragraphs, identify your selected Health Promotion topic, why you selected the topic, and why it is a community health concern.
- Demographic Data: In one or more paragraphs, compare the county, state, and national demographic data. Discuss how data are similar and how they are different.
- Epidemiological Data: In one or more paragraphs, compare the county, state, national, and world prevalence and mortality data. Discuss how data are similar and how they are different. Explain if the community health concern is improving or worsening. Provide a citation for at least one scholarly source.
- Community and Environmental Assessment Data: In one or more paragraphs, discuss the findings and trends in your community and environment assessment and how they relate to your practicum topic. Examine findings for evidence of the social determinants of health (SDOH). Identify two strengths and two areas for improvement within the community. Provide a citation for at least one scholarly source.
- Educational Material: In one or more paragraphs, discuss your educational material, how this material is appropriate for the community, and how it will help licensed healthcare professionals and community members improve health. Provide a citation for at least one scholarly source.
